Abstract
Skizofrenia merupakan gangguan kejiwaan dan kondisi medis yang dapat mempengaruhi fungsi otak manusia, mempengaruhi emosional, tingkah laku serta dapat mempengaruhi fungsi normal kognitif. Halusinasi merupakan salah satu gejala gangguan jiwa skizofrenia dimana seseorang akan mengalami perubahan pada persepsi sensori, merasakan sensasi yang palsu berupa suara, penglihatan, pengecapan, perabaan dan penciuman. Tujuan penulis mendapatkan gambaran dan pengalaman nyata tentang penerapan asuhan keperawatan pada Tn. R dengan masalah utama gangguan persepsi sensori: Halusinasi Di Ruang Teratai Rumah Sakit Umum Daerah dr. H Jusuf SK Tarakan. Pada pengkajian ditemukan keluhan utama klien mengatakan mendengar suara yang menyuruh klien berkata kasar dan ditemukan adanya kesenjangan yaitu beberapa tanda dan gejala yang ada pada teori tidak terdapat pada kasus Tn. R yaitu senyum sendiri dan menggerakkan bibir tanpa suara. Dari proses pengkajian diangkat 4 diagnosis keperawatan yang ada pada Tn. R yaitu: gangguan persepsi sensosri: Halusinasi pendengaran, Menarik diri: Isolasi sosial, Resiko perilaku kekerasan dan Defisit perawatan diri: Berhias. Semua intervensi telah dilakukan sesuai dengan strategi pelaksanaan pasien. Pada implementasi tidak didapatkan kendala karena klien sangat kooperatif. Evaluasi: setelah diberikan tindakan keperawatan selama 3x24 jam, didapatkan hasil semua diagnosis belum teratasi akan tetapi masalahnya berkurang. Diharapkan perawat mampu melanjutkan intervensi yang belum maksimal agar indikator dapat tercapai.

Schizophrenia is a psychiatric disorder and medical condition that can affect normal cognitive function, emotions, behavior, and brain function in people. Hallucinations are one of the symptoms of schizophrenia where a person will experience changes in sensory perception, feel false sensations in the form of sound, sight, taste, touch and smell. The author's goal was to get an overview and real experience about the application of nursing care to Mr. R with the main problem of sensory perception disorders: Hallucinations in the Lotus Room Regional General Hospital dr. H Jusuf SK Tarakan. In the study, it was discovered that the client's primary complaint was that he had heard a voice telling him to speak impolitely. It was also discovered that there was a gap, as some of the symptoms and signs listed in the theory had not been observed in the case of Mr R, who stands for silently moving his lips and smiling by himself From the assessment process, 4 nursing diagnoses were appointed to Mr R, namely: sensory perception disorders: auditory hallucinations, withdrawal social isolation, risk of violent behavior and self-care deficit embellishment All interventions were carried out according to the patient's implementation strategy. In the implementation there were no obstacles because the client was very cooperative. After receiving nurse care for three consecutive 24 hour periods, it was determined that while none of the diagnoses had been resolved, the problem had been lessened.
