ASUHAN KEPERAWATAN PADA PASIEN DENGAN DIAGNOSIS
KEPERAWATAN ISOLASI SOSIAL DI RUANG TERATAI RSUD dr. H.
JUSUF SK TARAKAN
ABSTRAK
Isolasi sosial merupakan salah satu masalah yang sering muncul pada penderita
skizofrenia. Isolasi sosial adalah kondisi terjadinya penurunan interaksi atau bahkan
tidak mampu untuk berinteraksi. Isolasi sosial biasanya muncul pada pasien dengan
gangguan kesehatan jiwa disebabkan oleh faktor predisposisi dan presipitasi.
Tujuan dari penulisan Laporan Tugas Akhir ini adalah untuk mendapatkan
gambaran mengenai penerapan asuhan keperawatan pada pasien skizofrenia dengan
masalah utama yaitu isolasi sosial. Metode penulisan Laporan Tugas Akhir ini
menggunakan metode studi kasus, dengan menggunakan cara wawancara. Penulis
melakukan asuhan keperawatan pada dua pasien skizofrenia dengan masalah utama
isolasi sosial. Hasil yang didapatkan tiga diagnosa yang ditegakkan pada pasien 1
yaitu isolasi sosial, harga diri rendah, dan defisit perawatan diri. Terdapat empat
diagnosa yang ditegakkan pada klien 2. Semua diagnosis tersebut di
implementasikan juga diamati secara mendalam dan pada pasien 1 hanya mampu
menjalankan strategi pelaksanaan isolasi sosial hingga sp ke tiga. Pasien 2 terlihat
lebih mampu menjalankan Sp isolasi sosial baik didampingi oleh perawat atau
secara mandiri. Kesimpulan terdapat diagnosa keperawatan yang sama pada kedua
klien yaitu isolasi sosial, harga diri rendah dan defisit perawatan diri dan terdapat
diagnosa berbeda yaitu diagnosa halusinasi pada klien 2 penulis tidak menemukan
kesenjangan antara teori, rencana keperawatan, implementasi keperawatan, dan
evaluasi dari studi kasus ini. Intervensi yang dilakukan telah disesuaikan dengan
kondisi pasien. Hasil dari penyusunan studi kasus ini diharapkan dapat menjadi
referensi dalam pelaksanaan asuhan keperawatan pada pasien dengan masalah
gangguan isolasi sosial.

NURSING ASSISTANCE FOR PATIENTS DIAGNOSED WITH SOCIAL ISOLATION IN THE HOSPITAL ROOM OF DR. H. JUSUF SK TARAKAN Abstract Social isolation is one of the problems that often arise in people with schizophrenia. Social isolation is a condition of decreased interaction or the inability to interact. Social isolation is common among patients with mental health issues due to predisposing and triggering circumstances. The goal of producing this Final Project Report is to provide an overview of the use of nursing care in schizophrenia patients with the primary issue of social isolation. This study used a case study method, which involved interviews. The author gives nursing care to two schizophrenia patients whose main issue is social isolation. The results revealed three diagnoses in patient 1, including social isolation, low self-esteem, and self-care impairments. All of these diagnoses were implemented and thoroughly watched, and client 1 could only carry out social isolation implementation tactics until the third session. Client 2 appears to be more competent in carrying out social isolation, whether accompanied by a nurse or independently.
