Latar belakang: kala i fase aktif adalah fase kritis persalinan yang menentukan keberhasilan proses kelahiran. pijat oksitosin merupakan intervensi non farmakologis yang diyakini dapat meningkatkan kontraksi uterus dan mempercepat pembukaan serviks, namun bukti klinis di fasilitas primer masih terbatas. tujuan: mengetahui efektivitas pijat oksitosin terhadap lama kala i fase aktif pada ibu multipara di ruang melati rsud malinau .metode: studi kasus dengan pendekatan kuantitatif–kualitatif pada satu ibu multipara (ny. r) kala i fase aktif (pembukaan 5 cm) .data dikumpulkan melalui wawancara, inspeksi, palpasi, auskultasi, dan observasi; pendokumentasian menggunakan format soap. prosedur: pijat oksitosin sesuai protokol selama fase aktif, pencatatan durasi kala i, kontraksi, djj, dan kondisi ibu janin. analisis dilakukan secara deskriptif. hasil: lama kala i fase aktif tercatat 3 jam 30 menit, lebih cepat dibanding rata rata fisiologis multipara. kontraksi menjadi lebih kuat dan teratur, djj stabil, dan tidak terjadi komplikasi maternal maupun fetal. ibu melaporkan penurunan kecemasan dan nyeri dapat ditoleransi tanpa analgesik farmakologis. temuan ini sejalan dengan penelitian mustaghfiroh & hesti (2022) dan pamungkas et al. (2024) yang menunjukkan pijat oksitosin memperpendek kala i dan menurunkan nyeri. kesimpulan: pijat oksitosin efektif mempercepat kala i fase aktif pada ibu multipara, aman, dan dapat diintegrasikan sebagai intervensi standar non farmakologis dalam asuhan kebidanan di fasilitas pelayanan primer. kata kunci: pijat oksitosin, kala i fase aktif, multipara, asuhan kebidanan, studi kasus.

Abstrak Indonesia

The first stage of labor is a crucial period in childbirth that significantly influences the outcome of the delivery process. oxytocin massage is a non-pharmacological strategy thought to stimulate uterine contractions and speed up cervical dilation, however, clinical data in primary care settings are sparse. the purpose of this study was to see how oxytocin massage affected the duration of the first stage of labor in mrs. r in the melati room of rsud malinau. this study used a case study method with a quantitative-qualitative approach to examine one multiparous mother (ms. r) during the first stage of labor (5 cm dilation) on june 3, 2025. data was obtained via interviews, inspections, palpation, auscultation, and observation documentation was done in soap style. the research procedure consisted of administering oxytocin according to protocol during the active phase while measuring the duration of stage i, contractions, fetal heart rate, and the mother's and fetus's conditions. the analysis was conducted descriptively. the study's findings revealed that the first stage of labor lasted three hours and thirty minutes, which was faster than the physiological norm for multiparas. contractions got stronger and more consistent, djj remained steady, and there were no maternal or fetal problems. the mother reported reduced anxiety and pain that could be managed without the use of pharmaceutical analgesics. research by mustaghfiroh & hesti (2022) and pamungkas et al. (2024) suggests that oxytocin massage can shorten the initial stage of labor and alleviate discomfort. the findings of this study indicate that oxytocin massage is beneficial in accelerating the active phase of labor in multiparous women, is safe, and can be utilized as a standard non-pharmacological intervention in obstetric care settings. keywords: oxytocin massage, active phase of labor, multipara, midwifery care, case study.
