
ASUHAN KEBIDANAN KOMPREHENSIF PADA NY. S DI PUSKESMAS JUATA KOTA TARAKAN

Pelayanan kebidanan komprehensif merupakan asuhan berkelanjutan yang diberikan mulai dari masa kehamilan, persalinan, nifas, hingga bayi baru lahir. asuhan ini bertujuan mendeteksi sejak dini kondisi ibu dan janin, mengantisipasi komplikasi, serta memberikan intervensi yang tepat. penulis memberikan asuhan kebidanan pada ny. s secara menyeluruh menggunakan pendekatan continuity of care (coc) di puskesmas juata, kota tarakan, mulai tanggal 10 april hingga 30 april 2025. kunjungan pertama dilakukan pada masa kehamilan trimester iii, dengan usia kehamilan 36 minggu 3 hari. hasil pengkajian menunjukkan kondisi ibu dan janin dalam batas normal tanpa komplikasi. asuhan persalinan dilakukan pada 26 april 2025 dengan usia kehamilan 38 minggu 2 hari. persalinan berlangsung kala i selama 5 jam 30 menit, kala ii selama 30 menit, kala iii selama 5 menit, dan kala iv dilakukan pemantauan selama 2 jam dengan hasil tanda-tanda vital stabil. bayi lahir cukup bulan, menangis spontan, warna kulit kemerahan, tonus otot baik, dan tidak terdapat cacat bawaan. setelah bayi lahir dilakukan penilaian sepintas, imd, serta pemberian vitamin k dan imunisasi hepatitis b. pada kala iv ditemukan perdarahan sekitar 150 cc dan robekan perineum derajat ii. telah dilakukan berupa penjahitan dengan anastesi dan pemberian edukasi perawatan luka perineum untuk mempercepat penyembuhan dan mencegah infeksi. kunjungan masa nifas dilakukan dengan hasil pemeriksaan menunjukkan uterus berkontraksi baik, terdapat lochea rubra dan luka jahitan perineum masih basah tanpa tanda infeksi. asuhan bayi baru lahir dilakukan saat kunjungan masa nifas. bayi dalam kondisi sehat, berat badan 3030 gram, panjang badan 50 cm, lingkar kepala 34 cm, lingkar dada 33 cm. tidak ada infeksi pada ta, serta refleks bawaan seperti moro, rooting, dan sucking baik. dengan demikian, dapat disimpulkan bahwa asuhan kebidanan komprehensif yang dilakukan pada ny. s berjalan secara fisiologis tanpa komplikasi berat, serta seluruh intervensi telah dilakukan sesuai standar kebidanan. pendekatan berkelanjutan ini terbukti efektif dalam menjaga kesehatan ibu dan bayi secara optimal.

Comprehensive midwifery care is a form of continuous care provided from pregnancy, childbirth, postpartum, to newborn care. this type of care aims to detect maternal and fetal conditions early, anticipate potential complications, and deliver appropriate interventions. the author provided comprehensive midwifery care to mrs. s using the continuity of care (coc) approach at juata public health center, tarakan city, from april 10 to april 30, 2025. the first visit was conducted during the third trimester of pregnancy, at 36 weeks and 3 days of gestation. the results of the assessment indicated that both the mother and fetus were in normal condition, with no complications found. labor care was carried out on april 26, 2025, at 38 weeks and 2 days of gestation. the labor progressed through four stages: the first stage lasted 5 hours and 30 minutes, the second stage 30 minutes, and the third stage 5 minutes. the fourth stage involved monitoring the mother for 2 hours, during which her vital signs remained stable. the baby was born full-term, cried spontaneously, had reddish skin, good muscle tone, and no congenital abnormalities were observed. after birth, an initial assessment was conducted, followed by early initiation of breastfeeding (eibf), administration of vitamin k, and hepatitis b immunization. during the fourth stage, approximately 150 cc of bleeding was observed, and a second-degree perineal tear was identified. suturing was performed under anesthesia, and the mother was educated on perineal wound care to support healing and prevent infection. the postpartum visit showed that the uterus was contracting well, lochia rubra was present, and the perineal stitches were still moist without signs of infection. newborn care was also conducted during the postpartum visit. the baby was in healthy condition, weighing 3030 grams, measuring 50 cm in length, with a head circumference of 34 cm and a chest circumference of 33 cm. there were no signs of infection at the umbilical cord site, and primitive reflexes such as moro, rooting, and sucking were present and functioning well. in conclusion, the comprehensive midwifery care provided to mrs. s progressed physiologically without any major complications, and all interventions were conducted in accordance with established midwifery standards. the continuity of care approach has proven effective in maintaining the optimal health of both mother and baby.
